REPLACEMENT DOCUMENTS
If you misplace or lose a document relevant to your motor vehicle and require a replacement, complete Application Form for Replacement Documents RF134 declaring which documents were lost and in what circumstances, and have the form stamped and witnessed at your garda station. Send your completed application form and the fee to the Motor Tax Department, Galway County Council, Prospect Hill, Galway.
Applications for replacement documents can only be accepted from the Registered Owner of the vehicle.
